The Coordinated Family Support Program

The purpose of the Empowering Families program is to improve the lives of 

families and individuals. The Empowering Families program helps individuals 

and their families by supporting them to become self-sufficient and to 

develop the skills to support their success to independence

Through one-on-one and peer support, staff educate, empower, and 

coordinate support services adults who reside in the family home and are 

tailored to the unique needs of individuals and their families taking into 

consideration their language and culture

The Empowering Families program provides the support needed for 

individuals and families to learn how to become self-sufficient and successful 

in meeting their needs. Families participating in the program receive 

comprehensive services through One-on-One Coaching with a Direct Support 

Coordinator (staff who work directly with the families).

ONE-ON-ONE COACHING sessions take place weekly throughout the calendar 

year, and frequency of sessions are based on the family’s needs and goals. 

Activities include:

• Resource Brokering

• Life Skills Development Education

• Diagnosis Supports

• Identify Direct Care Supports

• Service Delivery Support
